The California Primary Care Association (CPCA) launched a CPCA Medi-Cal Health Enrollment Navigators Project (Navigators Project) alongside the Department of Health Care Services (DHCS) that will award $17.75 million to subcontractors for work completed between December 1, 2023 and June 30, 2025. The funding is dedicated to community health centers (CHCs) and consortia that will assist with Medi- Cal outreach, enrollment, outreach, renewal, utilization, and troubleshooting efforts across California, which are considered “navigation services.”
Application Process & Deadlines
Our goal is to identify where allocations of funds for these Medi-Cal efforts are needed and where the funding would best be utilized for activities addressing the efforts described. The application is divided into two parts:
Part I
Application Closed September 26, 2023 | 5:00 PM PST
Identify interest and need for new funding for the Navigators Project, along with your most recent annual financial audit. Community health centers and consortia are encouragred to apply.
Part II
Application Now Open & Closes November 3, 2023 at 5pm PST
CPCA reviewed all applications submitted in part one of the application cycle and identified potential awardees according to the scoring criteria leveraged from DHCS. The Association provided preliminary funding notices and allocations to potential awardees so that they may build a narrative, work plan, and budget that aligns with the preliminary funding allocation. CPCA invites the potential awardees to complete the second part of the application.
The review team will utilize the information submitted in both parts one and two of the application to submit final funding recommendations to DHCS. DHCS will approve subcontracts with community health centers and consortia, including scope and budgets, prior to executing subcontracts.
Important Notes For Application Part Two:
- Only organizations who completed part one of the application and were selected as awardees based on scoring outcomes were invited to submit part two of the application. Only part two of the application submissions from organization identified as potential awardees will be considered for funding.
- Proposed funding allocations provided by CPCA to potential awardees may require applicants to scale down their proposed activities in part one of the application to create alignment between the funding available and scope of work.
- Both part one and two of the application must be completed by the applicant to be fully considered for funding and initiate a subcontract.
- Final applications (both Part I and Part II) will be shared with DHCS for funding approval. Overall responses will be summarized and used for reporting purposes.
- Subcontracts will be established with final subcontractors after DHCS approval.
