Harnessing Community Health Workers in Diabetes Prevention and Care Management

Community health centers play a critical role in diabetes prevention and management, yet many FQHCs face practical constraints related to funding, staffing, and reimbursement. This webinar provides a high-level overview of the National Diabetes Prevention Program (NDPP) and Diabetes Self-Management Education and Support (DSMES) with a focus on helping health centers assess feasibility and identify sustainable pathways for engagement, including how the core concepts of these programs can be integrated into existing health center offerings. Participants will learn about strategies for braiding funding streams to support CHW roles and increase billable visits. The webinar will distinguish between Medicare and Medi-Cal billing opportunities and how CHW roles support readiness for Value Based Care and Patient Centered Medical Home (PCMH) models. Participants will learn how integrating CHWs into diabetes care teams can strengthen patient engagement, improve outcomes, and address social determinants of health.
LEARNING OBJECTIVES
- Describe the role of NDPP and DSMES programs in addressing pre-diabetes and diabetes care, and how CHWs support program delivery and patient engagement.
- Identify strategies for braiding funding streams to sustain CHW roles in diabetes prevention and management, including opportunities to increase billable visits or apply core concepts of the program into existing health center services.
- Distinguish between Medicare and Medi-Cal billing pathways for NDPP and DSMES and strategies to support sustainability.
