The CPCA Health Center Controlled Network (HCCN)
Awarded in the Summer of 2025, the CPCA Health Center Controlled Network (HCCN) is a cooperative three-year agreement that supports health centers in strengthening their data, technology, and quality improvement capacity to enhance care delivery and promote positive health outcomes. Funded by the Health Resources and Services Administration (HRSA), CPCA’s HCCN brings together a wide variety of participating health centers (PHCs) that share a commitment to leveraging data and technology to improve health outcomes, streamline operations, and prepare for the future of value-based care.
Who We Serve
CPCA's HCCN represents over 900,000 patients spanning health centers in the central valley, central coast and northern regions of California.

Our Focus Areas
Through the HCCN, we provide training, technical assistance, and colleague-led learning opportunities focused on five key objective areas:

1. Data Management & Analytics
- Strengthening data infrastructure and analytics capabilities to drive performance improvment
- Supporting data validation, visualization, and storytelling to inform strategic decision-making

2. Data Exchange & Interoperability
- Expanding connections between health centers, hospitals, payers, and public health agencies
- Promoting use of Health Information Exchanges and Fast Healthcare Interoperability Resources (FHIR) based data sharing

3. UDS Modernization & FHIR Readiness
- Preparing systems for FHIR standards and enhanced reporting automation

4. Value-Based Care Readiness
- Building capacity for population health management, risk stratification, and quality improvement
- Supporting strategies that align clinical, financial, and operational goals under value-based payment models

- Promoting safe and fair use of artificial intelligence (AI) and emerging technologies

Our Approach
CPCA’s HCCN is designed as a learning network; a space where health centers collaborate, share best practices, and engage with colleagues and experts to solve shared challenges. Each participating health center works with CPCA to develop a tailored workplan aligned with network-wide objectives while reflecting their local priorities.
Participants also gain access to:
- Training and peer learning sessions with subject matter experts and partner organizations
- Technical assistance to improve reporting, quality improvement, and data use
- Collaborative platforms to foster communication and shared progress tracking across the network
