- Identify sustainable opportunities and challenges that health centers encounter while effectively meeting the needs of older adults
- Explore Medicare FFS, Medicare 340B, and Medi-Medi programs, along with a preview of D-SNP (Dual Special Needs Plans)
- Explore how CHCs can strategically leverage the potential of Medicare Advantage to enhance the quality of patient care and achieve improved outcomes
- Discuss how Medicare Accountable Care Organizations (ACOs) contribute to enhancing patient care quality and health outcomes
- Discuss the PACE Model, where an interdisciplinary team operates to deliver comprehensive medical and social services to older adults, facilitating independence and home-based care

click Proceed to Checkout. Lastly, complete all payment details and click, Submit. You will receive one
immediate email with a receipt of your order. Subsequent emails will follow

Cancellation Policy: Effective 11/1/21 - No refunds will be available for cancellations that are received within 10 business days before the start date of the event/training. To receive a maximum refund of 50% of the registration price prior to this deadline, all cancellations must be in writing and submitted by emailing training@cpca.org. Substitutions of registrations in lieu of cancellation is encouraged.
