Promoting Brain Health & Improving Dementia Support at CHCs

This webinar will explore the following:

(1) A local health department’s effort to promote brain health and improve the lives of people living with dementia, their caregivers, and their families. The discussion will highlight opportunities to prioritize brain health in the clinical setting and offer resources and tools to support individuals at risk for or impacted by dementia.

(2) A training and support program--Dementia Care Aware (DCA)--that empowers primary care teams to assess and address dementia. During this webinar you will learn about the new Cognitive Assessment Tool, while earning CMEs/CEs and MOC. The DCA enables the care team to conduct and bill for an annual assessment for older adults 65+; complete a quick check (5-10 min) on cognitive and functional symptoms and an assessment of the person’s support system; and begin a diagnostic assessment and a jump start to improve brain health.


LEARNING OBJECTIVES

  • Examine dementia from the public health perspective, particularly amid the unique challenges of the COVID-19 pandemic.
  • Learn about efforts to address dementia through multi-sectoral partnerships and early assessment (including how to conduct the cognitive health assessment: tools, scoring, and interpreting).
  • Identify strategies for improving the lives of people living with dementia, their caregivers, and their families.
  • Master billing requirements (149F) for DCA
